Рецидивные формы недержания мочи

В статье представлены результаты наиболее крупных исследований, посвященных изучению причин и тактики лечения рецидивной формы недержания мочи. Выделены две причины рецидива недержания мочи после имплантации субуретральных петель - неправильное позиционирование петли и недостаточная степень ее натяжения. На сегодняшний день отсутствуют четкие рекомендации по выбору тактики лечения больных рецидивной формой недержания мочи. Очевидна необходимость комплексного обследования данной категории больных, включающего гинекологический осмотр с проведением кашлевой пробы, уретроцистоскопию и комплексное уродинамическое исследование. На наш взгляд, больным рецидивной формой недержания мочи показано выполнение повторной петлевой операции, однако хирургическое лечение следует проводить не ранее 3 месяцев с момента предыдущей операции. По данным литературы позадилонная субуретральная петля обладает наибольшей эффективностью в лечении рецидива стрессового недержания мочи, что возможно объясняется тем, что позадилонная петля в наибольшей степени повторяет анатомию лобково-уретральной связки, нежели другие виды петель. В случае недостаточной компрессии предыдущей петля, расположенной в зоне средней уретры, необходимо выполнить ее рассечение. Однако при расположении петли в проксимальном или дистальном отделе уретры, ее рассечение не является обязательным. Согласно представленным данным, результаты лечения повторно оперированных больных оказались хуже, нежели в случае первичных пациенток, а интра- и послеоперационные осложнения встречались в большем проценте случаев. Поэтому крайне важным вопросом является создание в России тренинг-центров, позволяющих специалистам получать качественное обучение и перенять опыт и знания у ведущих специалистов страны.
