Проблема суицида при шизофрении: в фокусе внимания семья больного (обзор литературы)

В статье приведены современные данные о суицидальном риске при шизофрении с точки зрения теории биопсихосоциального подхода. Материалы были получены путём поиска в электронных базах данных: Pub Med, Scopus, Wiley Online Library, e-LIBRARY. Результаты: 44,3% пациентов с шизофренией имели хотя бы одну попытку самоубийства в течение жизни; до 4,9% погибают от суицида, при этом большая часть фатальных случаев приходится на первые годы болезни. Известно, что генетические факторы объясняют 43% суицидального поведения, остальные 57% - факторами окружающей среды. По данным 2005 г. достоверными факторами риска самоубийства при шизофрении являлись: депрессивные расстройства в анамнезе (Отношение шансов (ОШ)=3,03; 95% Доверительный интервал (ДИ) 2,06-4,46), попытки суицида в анамнезе (ОШ=4,09; 95% ДИ 2,79-6,01), злоупотребление наркотиками (ОШ=3,21; 95% ДИ 1,99-5,17), возбуждение или двигательное беспокойство (ОШ=2,61; 95% ДИ 1,54-4,41), страха психического распада (ОШ=12,1; 95% ДИ 1,89-81,3), плохое соблюдение режима лечения (ОШ=3,75; 95% ДИ 2,20-6,37) и недавние потери (OШ=4,03; 95% ДИ 1,37-11,8)...
