Кривая обучения лапароскопической и робот-ассистированной радикальной простатэктомии

Введение. Рак предстательной железы занимает лидирующие позиции в структуре онкологической заболеваемости и смертности. Радикальная простатэктомия в различных модификациях по-прежнему остается «золотым стандартом» в лечении неметастатических форм данной патологии. Однако, несмотря на большое количество публикаций, в настоящее время отсутствует консенсус относительно стандарта оценки как результатов оперативного лечения, так и продолжительности кривой обучения лапароскопической и робот-ассистированной радикальной простатэктомии. Цель исследования. Провести систематический обзор литературы, посвященной анализу кривой обучения лапароскопической и робот-ассистированной радикальной простатэктомии. Материалы и методы. Выполнен систематический обзор публикаций в базах данных PubMed и e-library, содержащих сведения о кривой обучения радикальной простатэктомии лапароскопическим (ЛРП) и робот-ассистированным доступами (РАРП). Результаты. За 30 летний период отобрано 49 статей: 17 - о ЛРП и 32 - о РАРП. Оценивались такие параметры, как время операции, объем кровопотери, продолжительность стационарного периода, положительный хирургический край и биохимический рецидив, частота осложнений, а также темпы восстановления функции удержания мочи и эрекции. Выводы. По-прежнему нет консенсуса относительно параметров кривой обучения ЛПР и РАРП. Стремление улучшить результаты и сократить время освоения диктует необходимость внедрения тренировочных программ и стандартизации хирургической техники.
