Комбинированное лечение операбельного рака молочной железы с использованием интраоперационной лучевой терапии

Цель исследования - изучить литературные данные, посвященные эффективности комбинированного лечения с использованием интраоперационной лучевой терапии (ИОЛТ) у больных операбельным раком молочной железы (РМЖ). Материал и методы. По теме найдено 110 зарубежных и российских публикаций (2000-21 гг.), представленных в базах данных Scopus, Pubmed, Elibrary и других, ключе-выми словами поиска явились: рак молочной железы, местный рецидив, интраоперационная лучевая терапия и методика облучения. Более значимые из них (45) включены в данный литературный обзор. Результаты. Лучевая терапия занимает важнейшее место в программе органосохраняющего лечения РМЖ. Многочисленными рандомизированными исследованиями за рубежом и в России было показано, что применение послеоперационной лучевой терапии резко уменьшает число локорегионарных рецидивов. Использование ИОЛТ как эффективного метода профилактики рецидивов по сравнению со стандартной послеоперационной адъювантной лучевой терапией является важным направлением в радиационной онкологии. Заключение. Таким образом, использование ИОЛТ в комбинированном лечении операбельного РМЖ должно быть дифференцировано и основано на совокупности клинико-морфологических факторов прогноза заболевания. Так, для различных молекулярных подтипов РМЖ характерны значительные отличия в патогенезе и ответе на проводимую терапию. Актуальным остается дальнейшее изучение эффективности ИОЛТ с целью выделения группы пациенток с абсолютными показаниями для проведения указанной методики.
