Эректильная дисфункция сосудистого генеза: комплексный подход к диагностике

Введение. Эректильная дисфункция (ЭД) - комплексное заболевание, ведущее к нарушению эрекции и неспособности совершения полноценного полового акта. Многими исследователями подтверждено, что наиболее частой причиной ЭД являются сосудистые нарушения, связанные, в первую очередь, с атеросклерозом. Цель. Анализ имеющихся литературных данных по методам диагностики ЭД сосудистого генеза, в том числе, с учетом состояния сердечно-сосудистой системы. Материалы и методы. Были проанализированы результаты поиска в научных базах данных Medline, EMBASE, Cochrane, Chinese BioMedical Literature Database, China National Knowledge Infrastructure, Elibrary. ru. Поиск в базах данных проводился по ключевым словам «диагностика эректильной дисфункции и атеросклероз», «эректильная дисфункция сосудистого генеза», «эректильная дисфункция и сердечно-сосудистые заболевания», «эндотелиальная дисфункция». На первом этапе было найдено 583 научных публикации, которые имели отношение к теме обзора. После детальной проверки достоверности источников, импакт-факторов журналов и последовательности изложения материала, непосредственно для цитирования были отобраны 63 наиболее подходящих по тематике научных публикации. Результаты. Первым этапом диагностики сосудистой ЭД является мониторинг ночных пенильных тумесценций. Далее проводится оценка факторов риска сердечно-сосудистых заболеваний и ЭД: сбор анамнеза по поводу повышенного артериального давления (АД), уровня физической активности, курения, измерение АД и массы тела пациента, анализ крови с определением липидного и гликемического профилей. Для диагностики эндотелиальной дисфункции (ЭНД) применяется определение поток-зависимой вазодилатации плечевой артерии с возможным определением скорости кровенаполнения после декомпрессии. После подтверждения ЭНД у пациентов с ЭД целесообразным является определение так называемого сосудистого возраста с помощью шкалы SCORE. Для дальнейшей тактики ведения пациента с ЭД и ЭНД рекомендована стратификация риска сердечно-сосудистых осложнений, связанных с сексуальной активностью. Выводы. Предложенный диагностический алгоритм позволит не только выявить сосудистый компонент ЭД на раннем этапе изменений, но и избежать фатальных сердечно-сосудистых осложнений путем назначения и рекомендации лечебных и профилактических мероприятий пациентам с бессимптомными сердечно-сосудистыми заболеваниями.
