Суицид - ведущая причина преждевременной смерти при шизофрении. Хотя суицидальные мысли могут быть на всех стадиях процесса, описаны отличия между суицидальным риском больных в первом эпизоде и хронических пациентов. В частности, риск выше на первом году расстройства и устойчиво снижено в последующем. Суицидальные мысли и попытки не редки у переживших субпсихотический опыт. К факторам риска суицида на ранних этапах шизофрении отнесены предыдущие суицидальные попытки, социальные (малая социальная поддержка, недостаток стабильных родственных связей, снижение статуса), психопатологические симптомы (бредовые идеи с настороженностью, императивные галлюцинации, дезинтеграция мышления, психомоторное возбуждение, негативные симптомы, депрессия, безнадежность) и злоупотребление ПАВ. Склонность к перфекционизму и хороший уровень критики лиц с недавними психотическими симптомами тесно связаны с высоким риском суицидальных попыток. Дисфункция префронтальной коры мозга может быть связана с суицидом на ранних стадиях шизофрении.
