Артериальная гипертензия у пациентов после коррекции коарктации и гипоплазии дуги аорты

В настоящее время хирургическая техника коррекции коарктации аорты существенно отработана, летальность является одной из самых низких в группе пациентов с врожденными пороками сердца. Однако частота развития артериальной гипертензии, сердечно-сосудистых и цереброваскулярных заболеваний по-прежнему высока, несмотря на отсутствие признаков рекоарктации и деформации аорты. Исследования показывают, такие пациенты имеют структурные особенности микроскопического строения стенки аорты (изменения эластично-коллагенового каркаса), функциональные нарушения (изменения функции барорецепторов) и макроанатомические (геометрические) особенности, обусловленные отличием от нативной формы дуги аорты после операции. В статье выполнен анализ возможных причин возникновения резидуальной артериальной гипертензии после хирургического лечения коарктации и гипоплазии дуги аорты. Высокая частота рецидива артериальной гипертензии остается важной проблемой у пациентов, перенесших коррекцию коарктации и гипоплазии дуги аорты, и требует глубокого изучения патогенеза, а также выбора оптимальной медикаментозной терапии.
