Treatment of posttraumatic gonarthrosis

According to the data of literature, posttraumatic gonarthrosis accounts for 9.4-25.5 % of cases among the degenerative diseases of the knee (Y.F. Kamenev et al., 1997; N.V. Sazonova, 2006). The process of rehabilitation of 143 patients (151 joints) at the age of 19-70 years (51.6±0.8 years) with posttraumatic gonarthrosis of all stages has been studied in Centre's clinic. The damages of menisci with their subsequent removal led to gonarthrosis development in 62 patients. Intraarticular fractures took place in 14 patients. Chronic microdamages and the damages of the knee capsular-and-ligamentous apparatus caused gonarthrosis development in 67 patients. In 96 patients (105 joints) gonarthrosis of mainly subcompensated and decompensated stages was accompanied by the disorder of limb biomechanical axis. The decompression-draining and decompression-correcting surgeries, worked out at the Centre, were performed in patients depending on their anatomic-and-functional disorders. Studying the long-term outcomes showed that positive results of treatment with 3-6-year favourable remission period were obtained in 96.7% of cases as a result of differentiated approach to surgical treatment of patients with posttraumatic gonarthrosis combined with adequate conservative therapy. The achieved anatomic-and-functional effectiveness of the surgeries is 15-20 % higher in comparison with the data of literature.
