Cold autoimmune hemolytic anemia is a rare disease associated with erythrocyte hemolysis at low temperatures with characteristic clinical symptoms. Studies of recent decades have made it possible to determine the relationship of this pathology with monoclonal proliferation of B-lymphocytes in the bone marrow. The article presents modern ideas about cold agglutinin disease: clinical manifestations, diagnostic criteria, treatment recommendations. A detailed description of the morphological picture of the bone marrow and the immunophenotype of monoclonal B cells is given. A clinical case of primary cold autoimmune hemolytic anemia with monoclonal proliferation of B-lymphocytes in the bone marrow is described.
