Влияние когнитивных функций на уровень суицидального риска у больных шизофренией

У пациентов с шизофренией ожидаемая продолжительность жизни значительно меньше в сравнении с общей популяцией, ассоциируясь с 13-15 потерянными годами потенциальной жизни. Более высокая смертность у пациентов с шизофренией связана в первую очередь с сердечно-сосудистыми заболеваниями и самоубийствами. В отдельных исследованиях обсуждается влияние когнитивных функций на риск суицида, однако, эти данные противоречивы. Цель исследования: оценить влияние когнитивных функций на уровень суицидального риска у больных шизофренией. Материалы и методы. Были обследованы 85 лиц (45 мужчин и 40 женщин в возрасте от 18 до 60 лет) с диагнозом шизофрении, верифицированным по исследовательским критериям МКБ-10. Все лица, включенные в исследование, прошли обследование с использованием шкалы краткой оценки когнитивных функций у пациентов с шизофренией (Brief Assessment of Cognition in Schizophrenia, BACS) в адаптированной русскоязычной версии. Оценка тяжести клинико - психопатологической симптоматики была выполнена с использованием Шкалы позитивных и негативных синдромов (Positive and Negative Syndrome Scale - PANSS) в адаптированной русской версии SCI-PANSS. Суицидальный риск оценивался по шкале безнадёжности Бека (Beck Hopelessness Scale - BHS). Математическая обработка проводилась с использоваием пакета прикладных программ STATISTICA for Windows (v.12.0). Результаты. В модели множественной линейной регрессии не было установлено значимого влияния отдельных компонентов нейрокогнитивного функционирования на уровень безнадёжности (p>0,05). Однако при использовании в модели множественной линейной регрессии метода прямого отбора переменных, было выявлено прямое влияние на уровень безнадёжности результата теста «Шифровка» (p=0,047), отражающего состояние внимания и скорости обработки информации, а также результата «Двигательного теста с фишками», не достигавшего, тем не менее, статистической значимости (p>0,05). Иные домены когнитивных функций, такие как вербальная и рабочая память, исполнительные функции, не оказывали значимого влияния на уровень безнадёжности (p>0,05). Помимо этого, в логистической регрессионной модели наиболее значимым фактором, определяющим умеренный и тяжелый уровень безнадёжности, являлся показатель «Двигательного теста с фишками» (p=0,014), также отражающий скорость обработки информации, а кроме того, состояние моторных функций. Прочие домены когнитивных функций не оказывали значимого влияния на уровень безнадежности и не влияли на прогностическую силу модели (p>0,05). Заключение. Установлено прямое влияние на уровень безнадёжности внимания, скорости обработки информации и моторных функций у больных шизофренией. Вместе с тем модели, полученные в ходе исследования, показали относительно небольшую прогностическую способность, что требует дальнейшего изучения влияния отдельных компонентов структуры нейрокогнитивного дефицита на суицидальный риск.
