Влияние хронической болезни почек на половую функцию у женщин

В статье изложены современные представления о гормональных дисфункциях у женщин репродуктивного периода, пре- и постменопаузы при хронической болезни почек (ХБП), включающих нарушения половой и репродуктивной функции, менструального цикла, снижение фертильности, повышение рисков невынашивания беременности при ее наступлении, а также влиянии почечно-заместительной терапии (ПЗТ) на половую и репродуктивную функции. Нарушения менстурального цикла характеризуются анаовуляторными циклами, может наблюдаться аменорея. В основе гормональных дисфункций лежит отсутствие пиков ЛГ и изменения концентрации эстрадиола в зависимости от фазы цикла, гипоэстрогенемия, гиперпролактинемия. В экспериментальных исследованиях показан нефропротективный эффект эстрогенов посредством влияния на синтез и высвобождению вазоактивных веществ, цитокинов и других факторов роста, которые ответственны за нарушение функции мезангиальных клеток, а также предотвращение действия трансформирующего фактора роста β-1 (ТФР-β-1), играющего роль в процессах гломерулосклероза.
