Современные тенденции использования лучевой терапии в лечении местнораспространенных (IIIA / N2 стадия) и ранних форм (I-II стадия) немелкоклеточного рака легкого, или роль лучевой терапии в лечении немелкоклеточного рака легкого на современном этапе

Двумя основными направлениями исследований эффективности лучевой терапии при немелкоклеточном раке легкого (НМРЛ), позволяющими надеяться на радикальность и удовлетворительные отдаленные результаты являются: 1) лечение местнораспространенного (IIIA/N2 ст.) НМРЛ и 2) лечение ранних форм (I ст.). В литературном обзоре показано, что послеоперационная лучевая терапия способна увеличить общую выживаемость, уменьшить частоту местных рецидивов при IIIA/N2 стадии НМРЛ, однако ее положительный эффект может нивелироваться повреждением окружающих структур. Очевидно, что решение задачи улучшения результатов ЛТ лежит на пути использования современной высокотехнологичной аппаратуры, совершенствования систем планирования, методик подведения доз облучения, режима фракционирования. Ни одно бимодальное лечение не показало преимуществ перед прочими сочетаниями. Некоторое улучшение выживаемости отмечено в ряде исследований с использованием трехмодального лечения, однако оно сопряжено с увеличением доли послеоперационных осложнений, особенно после пневмонэктомий. Гетерогенность группы IIIA/N2 НМРЛ, не до конца определенные критерии их операбельности представляют определенную сложность для корректного сопоставления результатов лечения, что требует дальнейшей дискуссии и изучения. В большинстве исследований недостаточно внимания уделено влиянию мультимодальных методик на качество жизни пациентов, что с учетом сопоставимых результатов лечения может иметь решающее значение в определении показаний и противопоказаний к нему. Пока не представляется возможным сделать обоснованные выводы об эффективности и безопасности стереотаксической лучевой терапии в качестве альтернативы хирургическому методу при лечении раннего (I стадия) НМРЛ, необходимо безотлагательное проведение рандомизированных контролируемых исследований с учетом предполагаемого высокого потенциала метода.
