Современные аспекты в лечении урологических заболеваний после трансплантации почки

Введение. Наиболее частые осложнения после трансплантации почки в отдаленном периоде включают обструкцию мочеточника, нарушение мочеиспускания, мочекаменную болезнь и пузырно-мочеточниковый рефлюкс. Эти осложнения традиционно устранялись открытыми хирургиче­скими вмешательствами, в настоящее время на первый план выступают минимально инвазивные методы. Материалы и методы. Выполнен обзор литературы преимущественно за последние 5 лет. Результаты. Стриктура мочеточниково-пузырного анастомоза является наиболее распространен­ным урологическим осложнением после трансплантации почки. Методом выбора в лечении является эндоуретеротомия, эффективность которой достигает 79%. Учащенное мочеиспускание днем и ночью часто отмечается среди реципиентов, причем эти изменения сохраняются спустя годы после пересадки. Выполнение трансуретральных оперативных пособий при инфравезикальной обструкции рекомендуется не ранее чем через 1­1,5 мес. после пересадки почки. Пузырно-мочеточниковый рефлюкс после трансплантации крайне распространенное явление. Наиболее изученным препаратом для эндоскопической коррекции является Deflux. У пациентов с начальными стадиями рефлюкса эффективность лечения составляет 90%. При нефролитиазе трансплантата с успехом используется ударно-волновая литотрипсия, перкутанная нефролитолапаксия, реже трансуре­тральная уретеролитотрипсия. В качестве альтернативы трансплантатэктомии больным с нефункционирующем трансплантатом и симптомами хро­нического отторжения может быть выполнена эмболизация трансплантата. Выводы. Последние достижения в области оборудования и техники позволили широко внедрить малоинвазивные технологии в лечении больных с трансплантированной почкой.
