Over the past decade it has increased the share of endovascular treatment of coronary heart disease (CHD) in the world. The choice of this treatment strategy of CHD, to counterbalance the surgical treatment in combination with conservative therapy caused the immediate efficacy and safety of the endovascular procedure to achieve adequate restoration of coronary blood flow in the majority of cases. Primary endovascular restoration of coronary blood flow has several advantages over thrombolytic therapy. There is evidence that reperfusion of the myocardium using the endovascular procedure is more than 95 % of patients with acute ST-segment elevation myocardial infarction (STEMI), then thrombolytic therapy restoration of blood flow is achieved only 7075 %. Endovascular method promotes effective limitation of the size of the damaged myocardium in the early stages of the onset of the disease, prevents the development of residual stenosis in the infarct-related artery (IRA) and pathological remodeling of the left ventricle (LV) and as a result, leads to a reduction not only in-hospital mortality, but and improved survival of patients in distant periods...

Tobacco use and exposure to tobacco smoke have serious negative consequences for health, the economy, the environment and the social sphere. People should be informed about these negative effects and the benefits of stopping tobacco use. Awareness of such negative consequences for the majority of tobacco users serves as a rather strong motivating factor in favor of refusing to use it. Promoting tobacco cessation and tobacco dependence treatment is an important element of a comprehensive, integrated tobacco control program. Health workers play a central role in identifying tobacco users, raising their awareness, promoting tobacco cessation, and providing support to tobacco users who want to quit tobacco addiction. There is clear scientific evidence that tobacco dependence treatment is effective, and it is a cost-effective medical intervention. Aim of the study was the analysis of the results of the WHO PEN clinical protocol implementation of healthy lifestyle education and advice on stopping tobacco use in patients with cardiovascular pathology in primary health care.

Research features and efficiency of antihypertensive therapy in patients with metabolic disorders

Cardiovascular disease (CVD) is an urgent problem of world and national medicine. The leading place among this pathology belongs to arterial hypertension (AH). The prognosis becomes even more unfavorable when combined with AH metabolic disorders. The number of patients with metabolic syndrome (MS) is projected to exceed 500 million by 2025. In patients with MS, the frequency of sudden death and CVD increases 2-4 times, the risk of coronary heart disease is 3-4 times higher, and the risk of ischemic stroke is 2 times higher. The risk of formation of type 2 diabetes increases 5-9 times. AH is one of the components of MS and is observed in more than half of patients. Target organs in such patients are affected much earlier and their changes are much more pronounced than in patients with hypertension without MS. We set a goal to study the effectiveness of treatment of arterial hypertension in patients with metabolic syndrome who were on inpatient treatment in the therapeutic Department of GBUZ RM «Republican clinical hospital №5» in Saransk in 2017-2018.

Conventional on-pump coronary artery bypass grafting operations in patients with compromised left ventricular (LV) systolic function are accompanied by high rates of morbidity and mortality. The wide spread in the last two decades of off-pump myocardial revascularization techniques in patients with low surgical risk showed the safety of such operations. In this regard, the question of the possibility and results of such operations in patients with high risk, in particular, with low contractility of the LV, is especially actual. Aim - to study the results of off-pump myocardial revascularization operations in patients with LV ejection fraction (LVEF) less than 45 %%.
